What is the Connected Care program?
Our APCM Connected Care program is designed for patients with chronic conditions such as diabetes, arthritis, and any other long-term health concern that requires extra support to stay in good health now and in the long-term.
When patients sign up for this program, they’re assigned a human care manager they can text or call with questions or concerns in between their regular primary care visits.
Our dedicated care managers are available Monday – Friday 8:30am – 4:30pm PST.

Who is eligible to participate?
Medicare patients age 65+ with one or more chronic health conditions are eligible to participate.

How much does the Connected Care program cost?
The Connected Care program costs between $0-$50/mo. Payments apply toward your deductible. Once the deductible is met, the cost would be free (or a monthly copay) depending on your insurance plan.

How to sign up
To sign up, call or text “start” to 425-517-1891 or ask your primary care provider for an APCM/Connected Care sign-up form.
Once enrolled, you’ll receive a text to confirm your participation. You can then begin getting direct support from one of our dedicated care managers.
Meet our Care Managers

Amanda
Amanda is a Registered Nurse dedicated to helping patients feel supported, informed, and confident in managing their health. She has over 10 years of experience in primary care, mainly at WWMG. This has given her a strong understanding of the unique needs of patients and the importance of accessible and compassionate care.
As part of the APCM Connected Care program, Amanda works closely with patients and their healthcare teams to provide personalized support and to coordinate care across their healthcare needs.

Christine
Christine is a Certified Medical Assistant with over 20 years’ experience in patient-centered care. At Western Washington Medical Group, she specializes in chronic care management, with a focus on supporting older adults and helping patients manage their ongoing health needs.
Her approach is solution-oriented, professional, and personal. She believes every patient deserves to feel heard, supported, and cared for. Christine brings extensive clinical experience, a genuine passion for helping others, and a commitment to making a positive difference in every patient’s healthcare journey.
